July 23, 2024: It was a celebration evening at the NeoCardioLab.  A big congratulation to Dr Pasinee Kanaprach for finishing her Neonatal Hemodynamics Fellowship at McGill. A big congratulation to Dr Silvia Nogara for finishing her research post-doctoral experience. A a big welcome to Dr Sariya Sahussarungsi for her start in the Neonatal Hemodynamics program at McGill University. We were joined by Daniela Villegas Martinez (NeoCardioLab Research Coordinator), Dr Nina Nouraeyan (Program Director of the Neonatal Perinatal Medicine program), Dr Michelle Ryan (Upcoming Director of the Neonatal Perinatal Medicine program), Dr Tiscar Cavalle (Director of the Pediatric Cardiology training program and member of our competency committee) and Dr Yalin Yin (Pediatric Cardiology, Pulmonary Hypertension and member of our Program Committee). Thank you all for this wonderful evening and annual rendez-vous.

July 15, 2024: I have added an example of a CDH-POCUS case. It includes a brief evaluation by lung ultrasound of the contralateral lung, a cardiac POCUS and a subxyphoid evaluation of the diaphragmatic motion of the right diaphragm (case of a left sided CDH).

July 14th, 2024: Inspired by the presentation of Dr McNamara (see here), I have created a section on important definitions in cardiovascular physiology. These highlight keep concepts that we consider when addressing cardiovascular physiology at the bedside. I hope to expand further this section in the upcoming months. Enjoy!

Abstract: Cardiac function is known to play critical role in the pathophysiological progression and ultimate clinical outcome of patients with congenital diaphragmatic hernia (CDH). While often anatomically normal, the fetal and neonatal heart in CDH can suffer from both right and left ventricular dysfunction. Here and we explore the abnormal fetal heart, early postnatal right and left ventricular dysfunction, the interplay between cardiac dysfunction and pulmonary hypertension, evaluation and echocardiographic assessment of the heart, and therapeutic strategies for managing and supporting the pathophysiologic heart and CDH. Further, we take a common clinical scenario and provide clinically relevant guidance for the diagnosis and management of this complex process.

Résumé : On sait que la fonction cardiaque joue un rôle essentiel dans la progression physiopathologique et l'évolution clinique ultime des patients atteints d'une hernie diaphragmatique congénitale (CDH). Bien que souvent anatomiquement normal, le cœur fœtal et néonatal en CDH peut souffrir d'un dysfonctionnement ventriculaire droit et gauche. Nous explorons ici le cœur fœtal anormal, le dysfonctionnement ventriculaire droit et gauche postnatal précoce, l'interaction entre le dysfonctionnement cardiaque et l'hypertension pulmonaire, l'évaluation et l'évaluation échocardiographique du cœur, ainsi que les stratégies thérapeutiques pour gérer et soutenir le cœur physiopathologique et le CDH. De plus, nous prenons un scénario clinique courant et fournissons des conseils cliniquement pertinents pour le diagnostic et la gestion de ce processus complexe.

June 27, 2024: It was an absolute pleasure to be part of the IPC 2024 (International Perinatal Collegium Meeting) in Quebec City from June 23 to June 27, 2024. I had the opportunity to showcase some of the work done by my laboratory (NeoCardioLab) and received valuable feedback and advice from leaders in neonatology. A big thanks to the organizing committee and Dr. Sylvain Chemtob for inviting me to this wonderful gathering of science and camaraderie. The event was filled with incredible research from passionate researchers in neonatology, stimulating brainstorming sessions, and the creation of significant professional connections. We also had the chance to experience the beauty of Quebec City and discover this wonderful part of my province and country. During the meeting, I presented the work done by my lab, highlighting the exceptional collaborations we've built over the years with colleagues within my institution, across the province, the country, and internationally. I also shared some early results from the TnECHO-Qc collaborative on the NORDIC-PREM project (more to come on that). Attached is the PDF of the slides from my presentation.

27 juin 2024 : Ce fut un immense plaisir de participer à l'IPC 2024 (International Perinatal Collegium Meeting) à Québec du 23 au 27 juin 2024. J'ai eu l'occasion de présenter une partie des travaux réalisés par mon laboratoire (NeoCardioLab) et j'ai reçu de précieux commentaires et conseils de la part de leaders en néonatalogie. Un grand merci au comité organisateur et au Dr Sylvain Chemtob de m'avoir invité à ce merveilleux rassemblement de science et de camaraderie. L’événement a permis d'être exposé à d'incroyables recherches par des scientifiques passionnées en néonatalogie, de séances de brainstorming stimulantes et de création de liens professionnels importants. Nous avons également eu la chance de découvrir la beauté de la ville de Québec et de découvrir cette merveilleuse partie de ma province et de mon pays. Lors de la réunion, j'ai présenté les travaux effectués par mon laboratoire, soulignant les collaborations exceptionnelles que nous avons bâties au fil des années avec des collègues de mon établissement, à travers la province, le pays et à l'échelle internationale. J'ai également partagé quelques premiers résultats de la collaboration TnECHO-Qc sur le projet NORDIC-PREM (plus à venir). Ci-joint le PDF des slides de ma présentation.

The experience was truly exceptional, offering a fantastic opportunity to interact with and learn from leading figures in neonatology. I extend my heartfelt thanks to Dr. Sylvain Chemtob for inviting me to this remarkable scientific gathering. Dr. Clyde Wright, a neonatologist and scientist at the Children's Hospital of Colorado, shared a compelling idea. He suggested exploring whether members of this community and users of the NeoCardioLab content have adopted new practices or enhanced their clinical approaches based on the information provided on this portal. Specifically, I'm interested in hearing whether the content has enriched your knowledge and influenced your neonatal care practices. I would greatly appreciate if you could take a few minutes to answer this survey.

June 23, 2024: Our section on Inotropes, Vasopressors and other cardiovascular medications is now updated. I have included a table with common dilutions that we use at our institution. I have also included calculators for rates based on the infusion dilution and desired dosage.

June 20, 2024: Today, I had the opportunity to teach the new transport nurses in neonatology at the Montreal Children's Hospital on neonatal cardiovascular physiology (white-board teaching). Lisa Carruthers, one of our most experienced transport nurses, is an expert in simulation and plays a crucial role in our NICU simulation program and the training and continuous education of our nurses, particularly in transport and resuscitation. Additionally, Lisa is an incredibly talented artist. She has created a stunning hand-drawn piece inspired by nature for the spring season. I would like to extend my heartfelt thanks to Lisa for all her contributions and for this beautiful gift. It is truly exceptional, and I wanted to share it with the entire NeoCardioLab community!

20 juin 2024 : Aujourd'hui, j'ai eu l'opportunité de former les nouvelles infirmières de transport en néonatologie à l'Hôpital de Montréal pour enfants sur la physiologie cardiovasculaire du nouveau-né (enseignement sur tableau blanc). Lisa Carruthers, l'une de nos infirmières de transport les plus expérimentées, est une experte en simulation et joue un rôle crucial dans notre programme de simulation en soins intensifs néonatals (USIN) ainsi que dans la formation et l'éducation continue de nos infirmières, en particulier en matière de transport et de réanimation. De plus, Lisa est une artiste incroyablement talentueuse. Elle a créé une magnifique œuvre dessinée à la main, inspirée par la nature, pour la saison printanière. Je tiens à exprimer ma profonde gratitude à Lisa pour toutes ses contributions et pour ce magnifique cadeau. C'est vraiment exceptionnel, et je voulais le partager avec toute la communauté NeoCardioLab !

June 19, 2024: Our annual summer concert at the Barrington-Janvier house was a total succes. A big thanks to Dr Annie Janvier and Dr Keith Barrington for inviting me (and my family) in their home. It was wonderful to be surrounded with friends and family to share this wonderful experience. Such a group full of talents that keep on inspiring me. Keith prepared a little video of the concert. June 19, 2024: Congratulations to Dr Audrey Hébert and colleagues for this great publication in Pediatric Research! In this retrospective study, we evaluated two approaches to the ductus arteriosus in preterm infants who survived to 36 weeks with established bronchopulmonary dysplasia, using a common definition. This analysis excludes those who did not survive, those who did not develop BPD and those who did not have a screening echocardiography at 36 weeks. We found that a conservative approach to PDA management yielded similar results regarding pulmonary hypertension (relatively low rates in both cohorts). This may be secondary to various different approaches throughout the early lifetime trajectory of these infants, suggesting that the occurence of PH is multi-factorial and complex, and not necessarily due to the ductal management approaches. It could also be that other management strategies (such as use of post-natal steroids or approaches to respiratory management) may participate in ductal flow modulation considering the two cohorts had the same rate of patent ductus at 36 weeks, despite them being selected to have BPD for entry criteria. 19 juin 2024 : Félicitations à Dre Audrey Hébert et collègues pour cette superbe publication dans Pediatric Research ! Dans cette étude rétrospective, nous avons évalué deux approches en lien avec la prise en charge du canal artériel chez des nourrissons prématurés ayant survécu jusqu'à 36 semaines avec une dysplasie broncho-pulmonaire établie, en utilisant une définition commune. Cette analyse exclut ceux qui n'ont pas survécu, ceux qui n'ont pas développé de dysplasie et ceux qui n'ont pas eu d'échocardiographie de dépistage à 36 semaines. Nous avons constaté qu'une approche conservatrice de la prise en charge du canal artériel donnait des résultats similaires en ce qui à trait avec l'hypertension pulmonaire (taux relativement faibles dans les deux cohortes). Cela peut être secondaire à diverses approches tout au long de la trajectoire de vie de ces nourrissons, ce qui suggère que la survenue de l'HTAP est multifactorielle et complexe, et n'est pas nécessairement due à l'approche de prise en charge du canal. Il se pourrait également que d'autres stratégies de prise en charge (telles que l'utilisation de stéroïdes postnatals ou des approches de prise en charge respiratoire) participent à la modulation du flux du canal, étant donné que les deux cohortes présentaient le même taux de persistance du canal à 36 semaines, bien qu'elles aient été sélectionnées pour avoir une dysplasia pour les critères d’entrée. June 19, 2024: New Case of the Month. Here, Dr Pasinee Kanaprach and Ms Thien-An Lam (under the guidance of Dr Cavalle-Garido and Dr Altit) present the case of a prenatal premature ductal closure with post-natal acute pre-capillary pulmonary hypertension. The case has also examples of echocardiographic clips / images, with explanation.

19 juin 2024 : Nouveau cas du mois. Ici, la Dre Pasinee Kanaprach et Mle Thien-An Lam (sous la direction du Dre Cavalle-Garido et du Dr Altit) présentent le cas d'une fermeture d'un canal artériel in-utero avec hypertension pulmonaire pré-capillaire aiguë postnatale. Le cas contient également des exemples de clips/images échocardiographiques, avec explication.

June 18, 2024: Our publication of our bubble CPAP implementation protocol is now published in Pediatrics "de Carvalho Nunes G, Barbosa de Oliveira C, Zeid M, Leone M, Mardakis S, Remmer E, Boyer J, Hailu E, Altit G, Beltempo M, Shalish W, Sant'Anna G. Early Bubble CPAP Protocol Implementation and Rates of Death or Severe BPD. Pediatrics. 2024 Jun 18:e2023065373. doi: 10.1542/peds.2023-065373. Epub ahead of print. PMID: 38887808." Following implementation, we have seen consistent improvement in our rate of chronic lung disease within a population of extreme premature newborns.

Suite à la mise en œuvre, nous avons constaté une amélioration constante de notre taux de maladie pulmonaire chronique dans une population de nouveau-nés extrêmement prématurés.

June 17, 2024: Congratulations to Dr Pasinee Kanaprach for graduating from the Neonatal Hemodynamics program at McGill University. Dr. Pasinee Kanaprach, a neonatologist from Thailand, joined our team in July 2023 after completing her training at Mahidol University and Ramathibodi Hospital in Bangkok, as well as practicing as the sole Neonatologist at the Chaophayayommarat hospital and Kasertsat University. Since her arrival at McGIll, Pasinee has quickly integrated into the healthcare teams at the Montreal Children's Hospital and the Jewish General Hospital. She has demonstrated remarkable enthusiasm for learning and an exceptional work ethic. Pasinee actively engages with the units, acquiring expertise in neonatal hemodynamics and neonatal echocardiography. Her passion for participation has led her to contribute to numerous projects and engage in teaching and academic activities. She has presented at international conferences and is working on three publications: two prospective studies and one review. Additionally, she contributes to the content of the NeoCardioLab platforms. Pasinee exemplifies qualities that make her an outstanding professional. She is caring, attentive to detail, efficient, hardworking, and always wears a smile. Her intelligence, proactive nature, and dedication to her work have set her apart. She actively participates in consultations and studies, earning appreciation from the NICU and Cardiology teams. Pasinee is also involved in teaching her peers through various platforms and animating the Neonatal Cardiology Academic Case Rounds. Pasinee is highly skilled in echocardiography and has developed expertise in performing neonatal hemodynamics assessments. Pasinee will continue her journey at McGill, joining the Neonatal Scholar program to engage in more research training. La Dre Pasinee Kanaprach, néonatologiste thaïlandaise, a rejoint notre équipe en juillet 2023 après avoir terminé sa formation à l'Université Mahidol et à l'hôpital Ramathibodi de Bangkok, ainsi qu'après avoir exercé en tant qu'unique néonatologiste à l'hôpital Chaophayayommarat et à l'Université Kasertsat. June 13, 2024: I had the distinct pleasure and honor of presenting "The Hemodynamics of Congenital Diaphragmatic Hernia" at the Targeted Neonatal Echocardiography Curriculum of the NHRC. I extend my heartfelt thanks to Dr. Dany Weisz and Dr. Danielle Rios for their exemplary leadership on the curriculum committee. As Dr. Weisz steps down from his role as co-chair, I am excited to assume this position and look forward to continuing to benefit from his invaluable advice and deep wisdom on the subject. I will be working with Dr Rios to keep this exemplary quality of training to the audience of trainees and neonatal practitioners. 13 juin 2024 : J'ai eu le plaisir et l'honneur de présenter « L'hémodynamie de la hernie diaphragmatique congénitale » dans le cadre du programme d'études d'échocardiographie néonatale ciblée du NHRC. 10 juin 2024 - Journal Club #23 de l'Incubateur Néonat. Dans cet épisode, Ben et Gabriel discutent de plusieurs sujets d'actualité en néonatologie. Ils évoquent d'abord la conférence Delphi Innovation qui aura lieu le 23 septembre et qui proposera cette année une participation virtuelle en plus de la présence sur place. Les sujets abordés iront de l'utilisation de l'échographie au chevet du bébé à l'intelligence artificielle. Ensuite, ils analysent une étude observationnelle de l'hôpital pour enfants Nationwide à Columbus, qui montre que la gestion de la dysplasie broncho-pulmonaire sévère est possible sans gazométrie sanguine de routine après 36 semaines, sans impact négatif sur les résultats cliniques. Le deuxième article discuté porte sur les bénéfices du peau à peau précoce chez les grands prématurés, qui ne semble pas augmenter les risques d'hémorragie intraventriculaire ou de sepsis, contrairement à certaines craintes. L'utilisation de la vidéo-laryngoscopie pour l'intubation en urgence des nouveaux-nés est ensuite abordée, avec des taux de réussite plus élevés dès la première tentative comparé à la laryngoscopie directe dans un essai randomisé. Enfin, les deux animateurs commentent une étude montrant l'impact croissant des vagues de chaleur sur le taux de naissances prématurées, soulignant les effets de l'environnement sur la santé périnatale. En conclusion, Gabriel mentionne aussi un congrès de cardio-néonatologie qui aura lieu début octobre à Montréal.

The presentation PDF, meticulously prepared by Ms. Karine Trudeau, a neonatal nurse at the Montreal Children's Hospital, and Dr. Sariya Sahussarungsi, a neonatal perinatal medicine resident at McGill University soon to be a Neonatal Hemodynamics fellow, offers an insightful exploration into the clinical presentation, pathophysiology, evaluation, and management of dilated cardiomyopathy during the neonatal period. June 7th, 2024 - Photo"mania". It was a pleasure to be in Vancouver for the Perinatal and Child Health Research Annual Meetings. I had the pleasure to participate to IMPACT and ENRICH. I had the opportunity to participate to fantastic workshops and learn a great bit from wonderful speakers! It was also wonderful to go to this conference with Parent-Partners Anthony Lucifero and Priyanka Kumar. Also, a big thanks to the team of BC Women's NICU for their wonderful hospitality. Dr Souvik Mitra and Dr Michael Castaldo gave me a fantastic visit of their beautiful state-of-the-art unit and of their neonatal hemodynamics program.

June 2nd, 2024 - It was a pleasure to host Dr Vinzenz Booz (Neonatologist from USZ – Universitätsspital Zürich - University Hospital Zurich) at the Montreal Children's Hospital. During his stay, Dr Booz was able to participate to some of the activities of the NeoCardioLab. We are hopeful that this will help establish a long-lasting partnership of research collaborations with our colleagues in Switzerland. Best return home!

May 11, 2024 - It was a pleasure and honour to present to the neonatal-perinatal medicine fellows of the Children's Hospital of Philadelphia on Wednesday, May 8th 2024. A big thanks to Dr William Corder and Dr Maria Fraga for inviting me. My teaching was on Cardiac Imaging - Basic Views (and image optimization). For this teaching, we filmed a live scan with a patient (would like to thank our family that accepted to be part of this!). Also a big thanks to my impromptu camera specialists: Dr Silvia Nogara, Dr Pasinee Kanaprach, Dr Paula Dias Maia and Ms Daniella Villegas Martinez. I have made now available the recording session with the explanations that were provided during the live scan. This outlines only a portion of the basic views that we obtain during a focused scan and some tips/tricks for image optimization. The video can be found in the TnECHO Section.

May 11, 2024 - A few pictures of the last few days. We had many wonderful visitors at the NeoCardioLab and at the Montreal Children's Hospital / McGill University Health Centre - Research Institute. Dr Beatrice Latal (Co-Head at Child Development Center, University Children's Hospital Zurich - Children's Hospital Zurich) gave wonderful presentations on the impact of congenital heart defects on the overall trajectory of development during childhood. She was invited by Dr Marie Brossard-Racine, principal investigator of abcdResearch from the MUHC-RI. Dr Vinzenz Boos (Neonatologist from USZ – Universitätsspital Zürich - University Hospital Zurich) presented to our group on APGAR scores and cord PH in the context of prematurity and congenital heart defects. Dr Silvia Nogara (post-doctoral student from Azienda Ospedaliera Universitaria Integrata in Verona, Italy; mentored by Dr. Benjamim Ficial) is also a visiting post-doctoral student spending 6 months at the NeoCardioLab to learn 3D-Echocardiography analysis. Dr Paula Dias Maia (neonatology fellow at University of Colorado; mentored by Dr Steve Abman) came to visit our hemodynamics program. Also in the picture: Dr Guilherme Sant'Anna (neonatologist and principal investigator of the Smart Hospital Project) and Dr Ana Sant'Anna (gastroenterologist at the Montreal Children's Hospital) who hosted a wonderful welcoming dinner. Dr Pasinee Kanaprach (fellow in our neonatal hemodynamics program) and Ms. Daniela Villegas Martinez (research coordinator at the NeoCardioLab). A big thanks to everyone for your visit and sharing your experience, knowledge, perspectives and insights to enrich our collective understanding of newborn care and newborn research.

May 11, 2024 - Shoutout to our incredible Congenital Diaphragmatic Hernia (CDH) multidisciplinary team at the Montreal Children’s Hospital (MUHC). This exemplary team, led by pediatric surgery director (and director of the CDH program) Dr. Pramod Puligandla, includes key specialists like Dr. Katryn Paquette and Dr. Louise Koclas from Neonatal Follow-up, Dr. Adam Shapiro from Pediatric Respiratory Medicine, Dr. Ana Sant’Anna from Gastroenterology, and pivotal nursing coordinator Sabrina Beusaigle. Our resident researcher, Dr. Alexandra Dimmer, is integral to the team's success. My role (G. Altit) involves screening for cardiovascular complications of CDH during follow-up, utilizing echocardiography at the clinic. Not pictured but equally crucial are our hemodynamics fellow and Daniela Villegas-Martinez, research coordinator at the NeoCardioLab. Heartfelt thanks to this wonderful team for providing exceptional care to CDH patients from prenatal life through their transition to adult medicine.

May 10, 2024 - It is a pleasure to announce the McGill Neonatal Conference 2024 chaired by Dr Guilherme Sant'Anna (Smart Hospital Project). The conference will be on Innovations and Technology in Neonatal Care: from the basics to the future. The invited international speakers are Dr Ola Didrik Saugstad, Prof John Rogers and Dr Debra Weese-Mayer. It will be a pleasure to present on Targeted Neonatal Echocardiography and Hemodynamics during this conference, which will be in person on September 17, 2024 at the Centre Mont-Royal in Montreal. May 2, 2024 - New case of the month, which highlights the case of April 2024. Submitted by Dr. Oung Savly (MD, FASE, Certified EACVI-CHD, pediatric cardiologist and Head of the division of pediatric cardiology/CICU/CHD cardiac surgery at Kantha Bopha Children’s Hospital, Phnom Penh, Cambodia). This case focuses on Anomalous Right Pulmonary Artery from Ascending Aorta. This rare anomaly can result in reactive pulmonary vascular constriction and pulmonary hypertension, leading to right ventricular failure and failure to thrive. Dr. Savly provides exemplary diagnostic imaging, including echocardiography and CT-scan reconstruction, accompanied by a brief discussion and post-repair images. We extend our gratitude to Dr. Savly for sharing this compelling case with the NeoCardioLab community.

April 25, 2024 - During the Neonatal Cardiology Academic Rounds at the Montreal Children's Hospital, Dr. Mohammed Alamer (Pediatric Interventional Cardiology Fellow) and Mrs. Laila Wazneh (Neonatal Nurse Practitioner) presented "Trans-Catheter Ductal Closure – What the NICU Should Know" to our group. This topic, a focal point of active research, involves considerable debate regarding the optimal timing, candidates, and methods for addressing patent ductus arteriosus (PDA) in premature infants in the NICU. They provided a thorough review of the practical considerations for trans-catheter ductal closure. The presentation is available here and in the Patent Ductus Arteriosus Section on Transcatheter Closure Considerations. This presentation was not intended to guide patient selection or promote a specific approach but offered insights into the realities and perspectives of interventional cardiologists concerning the procedure, the data available from published studies, and the aspects to monitor post-procedure if this method is chosen for certain patients. Our site practices a conservative approach to duct management in premature infants, pending further data from trials on mechanical closures using a trans-catheter method. We described a high spontaneous closure rate in our population with only about 10% of our cohort, who have residual shunts that do not close spontaneously, with follow-up in cardiology, and intervention considered for a handful of these patients from our cardiology colleagues just before or after discharge.

25 avril 2024 - Lors des séances académiques de cardiologie néonatale à l'Hôpital de Montréal pour enfants, le Dr Mohammed Alamer (fellow en cardiologie pédiatrique et interventionnelle) et Mme Laila Wazneh (infirmière praticienne néonatale) ont présenté à notre groupe "Fermeture ductale par cathéter - Que doit savoir la NICU ?". Ce sujet, au cœur de recherches actives, suscite de vifs débats concernant les candidats appropriés, le moment optimal et les méthodes pour traiter le canal artériel persistant (CAP) chez les prématurés en NICU. Ils ont effectué une revue approfondie des considérations pratiques concernant la fermeture ductale par cathéter. Cette présentation est disponible ici et dans la section du Canal artériel, spécifiquement dans l'approche de la fermeture par cathétérisme. Cette présentation n'avait pas pour but de fournir des directives sur la sélection des patients ni de préconiser une approche spécifique, mais elle a offert des perspectives sur la réalité et le point de vue des cardiologues interventionnels concernant la procédure, les données disponibles issues des études publiées et les éléments à surveiller après la procédure si cette méthode est choisie pour certains patients. Nous avons une approche conservatrice de la gestion du canal chez les prématurés, en attente de données supplémentaires issues d'essais sur les fermetures mécaniques par méthode trans-cathéter. Nous avons décrit un taux de fermeture spontanée élevé dans notre population avec seulement environ 10 % de notre cohorte, qui présentent des shunts résiduels importants qui ne se ferment pas spontanément, avec un suivi en cardiologie et une intervention envisagée par une poignée de ces patients de la part de nos collègues en cardiologie. juste avant ou après la sortie.

April 21, 2023 - Apical views updated in the TnECHO section. More specifically, added some material regarding the RV evaluation. Here we use Tissue Doppler Imaging superimposed on the M-Mode when acquiring TAPSE. TAPSE represents the movement of the base of the tricuspid annulus towards the apex of the heart during systole. TDI allows to outline the directionality of the motion of the myocardial wall, outlining systole and motion towards the probe and diastole with motion away from the probe. This allows delineation for measurement of TAPSE from the onset of the "blue-red" junction to the other junction of the "red-blue" zone of the M-mode. 

21 avril 2023 - Mises à jour des vues apicales dans la section TnECHO. Plus spécifiquement, ajout de matériel concernant l'évaluation du ventricule droit (VD). Ici, nous utilisons l'Imagerie Doppler Tissulaire superposée sur le Mode-M lors de l'acquisition du TAPSE. Le TAPSE représente le mouvement de la base de l'anneau tricuspide vers l'apex du cœur pendant la systole. Le Doppler tissulaire permet de tracer la directionnalité du mouvement de la paroi myocardique, délimitant la systole lorsque le mouvement va vers la sonde et la diastole lorsque le mouvement s'éloigne de la sonde. Cela permet de délimiter la mesure du TAPSE depuis le début de la jonction "bleu-rouge" jusqu'à l'autre jonction de la zone "rouge-bleu" du Mode-M.

April 12, 2024 - It was an honour and privilege to present at the 3rd International Symposium on NIRS. I presented some of the work that we do at the NeoCardioLab on NIRS in the research of infants with congenital heart defects. The slides are available here and in the NIRS section. A special thanks to the organizing committee of the POCUSNEO team for their warm invitation and for putting together such a wonderful virtual symposium. April 11, 2024 - Training day at the NeoCardioLab and NeoBrainLab of Dr Pia Wintermark. We would like to thank Mrs Jessica Simoneau, Imaging Technologist and teacher in sonography, for her guidance and training during this day. We are preparing a study evaluating many organs of previous graduates of the NICU.  This workshop centered on imaging of the kidneys. Thanks to Mrs Simoneau for her time and diligence in outlining the technique to obtain a rigorous methodology to visualize kidneys, obtain their measurements, and obtain the Dopplers. More to come on this project!

11 avril 2024 – Journée de formation au NeoCardioLab et NeoBrainLab du Dr Pia Wintermark. Nous tenons à remercier Mme Jessica Simoneau, technologue en imagerie et enseignante en échographie, pour son encadrement et sa formation lors de cette journée. Nous préparons une étude évaluant de nombreux organes d'anciens diplômés de l'USIN. Cet atelier était centré sur l'imagerie des reins. Merci à Mme Simoneau pour son temps et sa diligence à exposer la technique permettant d'obtenir une méthodologie rigoureuse pour visualiser les reins, obtenir leurs mesures et obtenir les Dopplers. April 10, 2024 - It is a pleasure to join the ENRICH program and the IMPaCT (Increasing capacity for Maternal and Paediatric Clinical Trials) program, which "is a Clinical Trials Training Platform funded by the Canadian Institutes for Health Research". This is through the Canadian ENRICH program (Empowering Next-generation Researchers In perinatal and Child Health). I highly encourage Canadian scientists in the perinatal and pediatric research space to register to these programs to increase skills and knowledge in research methods, as well as a networking space. April 9, 2024 - We had a wonderful time at the "Tiny Survivor" event hosted by the Just for Kids Foundation. Alongside Dr. Jarred Garfinkle and Dr. Michelle Ryan, we attended this fundraising evening held at Espace Gare Viger. The proceeds from the event will go towards funding equipment for the neonatal intensive care unit at the Montreal Children's Hospital. I would like to extend a heartfelt thank you to the Just for Kids Foundation for their ongoing support of the NICU, as well as to all the participants and donors. Your contributions make a significant difference. It is worth noting that the Just for Kids Foundation supported the NeoCardioLab in acquiring the echocardiography machine in 2018.

9 avril 2024 - Nous avons passé un merveilleux moment à l'événement "Tiny Survivor" organisé par la Fondation pour enfants seulement. Aux côtés du Dr. Jarred Garfinkle et du Dre. Michelle Ryan, nous avons assisté à cette soirée de collecte de fonds qui s'est déroulée à l'Espace Gare Viger. Les bénéfices de cet événement serviront à financer du matériel pour l'unité de soins intensifs néonatals de l'hôpital de Montréal pour enfants. Je tiens à remercier personnellement la Fondation pour enfants seulement pour son soutien continu à l'unité de soins intensifs néonatals, ainsi que tous les participants et donateurs. Vos contributions font une grande différence. Il est également important de souligner que la Fondation pour enfants seulement a soutenu le NeoCardioLab dans l'acquisition de la machine d'échocardiographie en 2018.

Mrs. Milette is an expert in developmental care. She is the president of "Developmental Care" and a pioneer in Quebec and the French-speaking world in neonatal developmental care. April 6th, 2024: I have included an illustrative example showcasing profound biventricular dysfunction in the LV function section. You can access it here. The majority of echocardiographic views are accompanied by explanatory notes. While a comprehensive assessment including Tissue Doppler Imaging and other Dopplers hasn't been shared, the provided images effectively highlight the substantial biventricular dysfunction with pertinent metrics delineated.

April 6th, 2024 - I have made our pulmonary hypertension screening in extreme preterm infants and management guidelines available here. They were developed in collaboration with the TnECHO-Quebec Collaborative. You may also find them in the Bronchopulmonary Dysplasia - Pulmonary Hypertension section. These guidelines were adapted from the collaborative's work to streamline screening and management across multiple sites within our province. The guidelines provide a screening algorithm. Local guidelines should always be tailored to your own reality, multidisciplinary team, and population.

6 avril 2024 - J'ai rendu disponibles ici nos directives de dépistage de l'hypertension pulmonaire chez les nouveau-nés extrêmement prématurés, ainsi que notre prise en charge. Elles ont été élaborées en collaboration avec le TnECHO-Québec Collaborative. Vous pouvez également les trouver dans la section Bronchopulmonary Dysplasia - Pulmonary Hypertension. Ces directives ont été adaptées du travail collaboratif pour rationaliser le dépistage et la prise en charge dans plusieurs sites de notre province. Les directives fournissent un algorithme de dépistage. Les directives locales doivent toujours être adaptées à votre propre réalité, à votre équipe multidisciplinaire et à votre population.

April 4, 2024: On April 3rd, 2024, I had the pleasure of presenting on Point of Care Ultrasound (POCUS) at the Montreal Children's Hospital during the Pediatric Grand Rounds. It was an honor to share the stage with Dr. Jade Séguin (Pediatric Emergency Medicine) and Dr. Samara Zavalkoff (Pediatric Critical Care Medicine). Our presentation is available here in PDF.  Since 2019, we have established the MCH POCUS Standing Committee, tasked with overseeing and guiding the implementation of POCUS within our pediatric framework. We conduct workshops for trainees, incorporating simulation for enhanced learning. This initiative has proven to be immensely successful, owing to the collaborative efforts of our multidisciplinary team. We take pride in the diverse expertise brought forth by our team, consisting of physicians and allied healthcare professionals. In 2024, we are excited to announce that we will be extending our course offerings to the wider community. Stay tuned for further updates and information!

C'était un honneur de partager la scène avec la Dre Jade Séguin (Médecine d'urgence pédiatrique) et la Dre Samara Zavalkoff (Médecine de soins intensifs pédiatriques). March 29, 2024: This year, I am a member of the organizing committee for the Delphi Conference on neonatal innovation! This conference is organized by the team of the Incubator Podcast. "The Delphi conference is an annual meeting that focuses on the latest innovative thinking in medicine and more specifically neonatal care. Delphi aims to create a forum where novel ideas can be shared by like-minded individuals to spark innovative thinking in neonatology. Members of the Delphi community are what Steve Jobs referred to as the ones who see things differently. We hope this forum can become the place where innovative minds can exchange thoughts and ideas about the path our field will take in the near future." You can find here the full agenda.

March 14, 2024 - It was truly an honour and pleasure to have Dr Benjamin Courchia from The-Incubator Podcast (English and French) and from Delphi Innovation Conference visiting our division at the Montreal Children's Hospital and McGill University to visit our neonatal unit, give neonatal rounds on "Neonatal Innovation", and visit the NeoCardioLab. His expertise and passion for neonatology shone through as he shared his thoughts and experiences on various topics, including academia, global health, neonatal care on a global scale, the role of social media in medicine, and application development. We are very grateful for his presentation and insight on the future of neonatal medicine!

March 7th, 2024: Thank you to the Montreal Children's Foundation for the constant support to the NeoCardioLab. We received a new S9-2 probe to improve cardiac image acquisition at the NeoCardioLab. Thanks to the generous donations towards the MCH foundation that continue to support the advancement of the science and care we provide at our institution.

March 1st 2024: The Department of Pediatrics, Division of Neonatology at McGill University and the Montreal Children’s Hospital invites applicants for a full-time position as a member of the Contract Academic Staff, as an academic clinician neonatologist, at the rank of Assistant or Associate Professor (Clinical). The successful candidate’s mandate will include a major clinical and educational participation in our neonatal hemodynamics and TnECHO (targeted neonatal echocardiography) program, while ensuring its ongoing development.  In addition, the candidate may be expected to eventually take on the directorship of the neonatal hemodynamics fellowship training program. Of note, this can be an intent to pursue further training in TnECHO-Neonatal Hemodynamics prior to permanently joining our division. The successful candidate will be expected to contribute to our interdisciplinary education mission, including the training of neonatal nurse practitioner students, medical students, and residents. Other responsibilities may include participation in academic and administrative committees, such as the fetal diagnostic and treatment group and the neonatal cardiology academic rounds. Of note, bilingual language skills (French and English) are preferred, or a commitment to acquire the necessary language skills to practice medicine in Quebec. February 20, 2024: It was truly an honor to have the opportunity to present at the Bankgok International Neonatology Symposium 2024. The pre-congress workshop focuses on Targeted Neonatal Echocardiography (TNE) and was held on February 19-20, 2024. I extend my heartfelt gratitude to the organizing committee for their invitation, hospitality and meticulous planning. The quality of the talks throughout the event are outstanding. The first day of the symposium encompassed immersive sessions on hands-on echo skills and engaging case discussions. February 20 promises to delve deeper into pivotal topics of TNE and Neonatal Hemodynamics (NH). Here are some pictures of the first day! My talks are available here in the format of PDF.

Le 20 février 2024 : Ce fut vraiment un honneur d'avoir eu l'opportunité de présenter lors du Bankgok International Neonatology Symposium 2024. L'atelier pré-congrès portant sur l'échocardiographie néonatale ciblée (TNE) s'est tenu les 19 et 20 février 2024. January 27, 2024 - We are pleased to announce that the EMBLEM study - Financed by CIHR (PI: Dr. Sherri Katz) - has been launched at the NeoCardioLab, Montreal Children's Hospital - MUHC-RI. First MCH participant and first Canadian participant in this multi-centric Canadian observational study evaluating pulmonary MRI, echocardiography and lung ultrasound to improve our definition of bronchopulmonary dysplasia in preterm infants. Thanks to the wonderful team at the NeoCardioLab and MCH for making this possible! For more information on EMBLEM - click here. We obtained parental consent for this video inaugurating the study - we would like to thank them for their generosity in participating and allowing us to take these important images of the moment!

27 janvier 2024 - Nous sommes heureux d'annoncer que l'étude EMBLEM - Financée par les IRSC (IP: Dre Sherri Katz) - a été lancée au NeoCardioLab de l'hôpital de Montréal pour enfants - IR-CUSM. Premier participant à l'HME et premier participant canadien à cette étude observationnelle canadienne multicentrique évaluant l'IRM pulmonaire, l'échocardiographie et l'échographie pulmonaire pour améliorer notre définition de la dysplasie broncho-pulmonaire chez les nourrissons prématurés. January 16th, 2024: We are delighted to present the Case of the Month for January 2024 - a case featuring Twin Twin Transfusion Syndrome with Hypovolemia. This case underscores the significance of vigilantly monitoring fluid status in newborns following a TTTS setup. This case was prepared by Dr. Pasinee Kanaprach, a fellow in neonatal hemodynamics at McGill University. Le 16 janvier 2024 : Nous sommes ravis de vous présenter le Cas du mois de janvier 2024 - un cas mettant en lumière le syndrome transfuseur-transfusé avec hypovolémie. Ce cas souligne l'importance de surveiller attentivement le statut hydrique des nouveau-nés après un STT. Ce cas a été préparé par Dre Pasinee Kanaprach, fellow en hémodynamie néonatale à l'université McGill. January 13th, 2024: On January 12th, we had the pleasure of visiting the Maude Abbott Medical Museum. I extend my gratitude to Dr. Marie J. Béland, who generously guided us through this visit and conducted an exceptional teaching session for our fellow in Neonatal Hemodynamics at McGill University, as well as those in Pediatric Cardiology Advanced Imaging and Pediatric Interventional Cardiology. Dr. Béland, a member of the Board of Directors and a founding member of the International Society for Nomenclature of Paediatric and Congenital Heart Disease (ISNPCHD), is a Pediatric Cardiologist with an unparalleled depth of knowledge. She served as my teacher throughout medical school, residency, and fellowship. She has expertise in electrophysiology, and has comprehensive knowledge/experience of fetal and pediatric cardiology (she knows everything!). Currently a H-KAP fellow (Helen Karounis Agora Program) at McGill University, Dr. Béland generously shared her time and expertise. The learning experience with Dr. Béland was enriching and incredible as she provided an overview of the journey towards a unified nomenclature in congenital heart defects, covering basics of cardiac anatomy/morphology and appropriate terminology. We also had the opportunity to review some preserved anatomy cases at the Maude Abbott Medical Museum. 13 janvier 2024 : Le 12 janvier, nous avons eu le plaisir de visiter le Musée médical Maude Abbott. J'exprime ma gratitude à la Dre Marie J. Béland, qui nous a généreusement guidés lors de cette visite et a animé une séance d'enseignement exceptionnelle pour notre fellow en hémodynamie néonatale à l'Université McGill, ainsi que pour ceux en imagerie avancée en cardiologie pédiatrique et cardiologie pédiatrique interventionnelle. La Dre Béland, membre du conseil d'administration et membre fondateur de la Société internationale de nomenclature de la cardiologie pédiatrique et congénitale (ISNPCHD), est une cardiologue pédiatrique avec une profondeur de connaissance inégalée. Elle a été mon enseignante tout au long de mes études de médecine, de ma résidence et de ma spécialisation. Expert en électrophysiologie, elle possède une connaissance et une expérience complètes de la cardiologie fœtale et pédiatrique (elle sait tout !). Actuellement, elle est "fellow" H-KAP (programme Helen Karounis Agora) à l'Université McGill, et la Dre Béland partage généreusement son temps et son expertise. L'expérience d'apprentissage avec la Dre Béland a été enrichissante et incroyable, car elle a présenté un aperçu du cheminement vers une nomenclature unifiée des malformations cardiaques congénitales, en abordant les notions de base de l'anatomie/morphologie cardiaque et de la terminologie appropriée. January 13th, 2024: More on Dr Abbott: Maude Abbott (1869–1940) was a Canadian physician, pathologist, and medical historian. She is best known for her significant contributions to the field of pathology, particularly in the study of congenital heart disease. Abbott played a crucial role in establishing the field of pediatric pathology in Canada. Abbott's work in documenting and classifying congenital heart defects has had a lasting impact on the understanding and treatment of heart conditions in children. Her influential work, "Atlas of Congenital Cardiac Disease," published in 1936, remains an important reference in the field. In addition to her contributions to medicine, Maude Abbott was a trailblazer for women in the sciences. She faced challenges and discrimination as a woman pursuing a career in medicine during a time when it was not common. Despite these obstacles, she became the first woman to be appointed as a professor at McGill University in Montreal. Maude Abbott's legacy extends beyond her lifetime, and she is remembered for her pioneering work in pathology and her role in advancing medical knowledge, especially in the area of congenital heart disease.

Elle est surtout connue pour ses contributions significatives dans le domaine de la pathologie, en particulier dans l'étude des malformations cardiaques congénitales. Abbott a joué un rôle crucial dans l'établissement de la pathologie pédiatrique au Canada. Son travail dans la documentation et la classification des malformations cardiaques congénitales a eu un impact durable sur la compréhension et le traitement des affections cardiaques chez les enfants. Son œuvre influente, "Atlas of Congenital Cardiac Disease," publiée en 1936, demeure une référence importante dans le domaine. January 12th, 2024: This year, I was invited to take part in a holiday concert at the Barrington-Janvier family's home, where every member of their super-musician family participates! Dr. Annie Janvier and Dr. Keith Barrington are exceptional friends. They are brilliant neonatologists, researchers, and academicians (and super talented musicians!!). More than that, they are fantastic parents, and they have a beautiful family. I would like to thank them for pushing me out of my comfort zones and welcoming me into their family for this beautiful holiday concert! Here is the results :) (with their permission to publish online).

12 janvier 2024 : Cette année, j'ai été invité à participer à un concert pour la saison des fêtes chez la famille Barrington-Janvier, où chaque membre de leur famille de super-musiciens participe ! La Dre Annie Janvier et le Dr Keith Barrington sont des amis exceptionnels. Ce sont des néonatologistes, des chercheurs et des universitaires brillants (et des musiciens super talentueux !!). Plus que cela, ce sont des parents fantastiques, et ils ont une magnifique famille. Je tiens à les remercier de m'avoir poussé hors de ma zone de confort et de m'avoir accueilli dans leur famille pour ce magnifique concert de la saison des fêtes ! January 12th, 2024: We updated the normative values section and included a section on neonatal hypertension. As well, updated the content on the inotropic support and added antihypertensive medications.

January 10th, 2024: New wonderful presentation prepared by Laila Wazneh and Kathryn Jones, both neonatal nurse practitioners at the Montreal Children's Hospital. This presentation was prepared in the context of the neonatal cardiology academic case rounds at McGill University, in collaboration with Dr Tiscar Cavalle (pediatric cardiologist at the Montreal Children's Hospital). The presentation was on Tetralogy of Fallot. You can now access the slides, as well as our local algorithm for the management of TET spells through the section on Tetralogy of Fallot. 

10 janvier 2024 : Nouvelle magnifique présentation préparée par Laila Wazneh et Kathryn Jones, toutes deux infirmières praticiennes néonatales à l'hôpital de Montréal pour enfants. Cette présentation a été préparée dans le cadre des séries de cas académiques en cardiologie néonatale à l'Université McGill, en collaboration avec la Dre Tiscar Cavalle (cardiologue pédiatrique à l'hôpital de Montréal pour enfants). January 9th, 2024: In the context of our collaborations in the SARABI study and the At Heart of the Matter study, we created modules in french and english to train in knobology (for our echocardiography machine), as well as for the sequence of image acquisition in adults that we recruit within these projects. These modules are for training the imaging technologists that are integrating our team for the acquisition of images in these participants. The videos are available here, and on the respective page of these studies.

January 9th, 2024: Congratuations to Dr Sariya  Sahussarungsi who presented her project at the CHHD Work-In-Progress. Her project is on the Feasibility of 3D-Echocardiography for LV performance and dimensions in the immediate postnatal neonatal period - a propspective study.  Her project has also been approved for presentation at PAS in Toronto (2024). 

9 janvier 2024 : Félicitations au Dre Sariya Sahussarungsi qui a présenté son projet au CHHD Work-In-Progress. Son projet porte sur la faisabilité de l'échocardiographie 3D pour l'évaluation de la performances et les dimensions du VG dans la période néonatale postnatale immédiate - une étude prospective. Son projet a également été approuvé pour présentation au PAS à Toronto (2024).
